Enrolling at Ilam Medical

Thank you for choosing to enrol with Ilam Medical Centre. Please read through the following information. If you have any questions, call our reception team to discuss.

Checklist for enrolment:

1. Are you eligible?   Refer to the 2nd page of our enrolment form or for more information go to the Ministry of Health website
2. Are you living within our Enrolment Zone, see map
3. Read our Terms of trade
4. Download and complete the following:
   Enrolment form & New patient Health questionnaire

•  Please note that every patient over the age of 16 is required to sign their own enrolment form.
•  Bring completed forms along with photographic ID into the practice. Please do not email completed forms as original ID must be sighted by our staff in the practice. You will also need to bring in evidence of your current address to confirm you are living within our enrolment zone.  Power, rates or telephone accountetc.

•  NZ citizens can bring one type of photographic ID eg, Drivers licence, Student ID, 18+ card or passport.
